All non-West guest forms for prom will be due this Fri., April 5 at 3:30 p.m. These forms can be turned into the Student Activities anytime before then.

The applications were sent through email and can be found there. All guests must be under 21 years old and need their dean’s signature if they are still in high school and both people’s IDs need to be present on the night of the dance. Be sure to get this done sooner than later.
